Title 1 and Voucher Pre-k schools may apply for a grant to cover costs of a field trip. Applicable while grant funds are available and if school meets grant criteria. Formal request must be made on the agency letterhead and signed by an authorized representative of the agency. The request must contain the following information:

• Name and address of the agency
• Name of authorized contact and address
• Preferred field trip or outreach program:
• Preferred date of the field trip (provide 3 dates):
• Grade level:
• Number of classes:
• Number of students:
